What to Expect:

To best prepare us for your visit, we request that some important background information be received prior to your appointment. This includes your completed registration, medical history. Receipt of these forms as well as pertinent x-rays or test results from your doctor or dentist before your appointment is necessary to ensure we fully address your concern. Patient registration and medical history forms are available from our office or may be downloaded below.  Please return these forms to our office prior to your appointment.

During the first visit, we take the time to discuss your concerns and history of your symptoms. The length of the first visit is dependent on individual needs and may take up to an hour. This initial process is an important step in developing a diagnosis and management plan.

After your first appointment, your diagnosis and management plan may involve continued contact with your physician, dentist, and other medical specialists. The most important part of this process is continued communication with you. This is your management plan and it is important to us that you feel involved in the process.

Follow-up visits may be needed to evaluate for any changes or fine-tune a current management plan.

Financial Information:

The cost of your visits may vary depending on your individual needs and treatment plan. Please contact our office if you have specific questions.

We accept patients with or without insurance.  We accept Visa, Mastercard, American Express, Care Credit, personal checks and cash.

We are happy to see individuals with any insurance plans and those without insurance.  We are contracted providers only with Tricare.  Some insurance companies may allow coverage within their guidelines.  With the exception of patients with Tricare insurance, payment in full will be collected at the time of service.  We will provide you with a detailed receipt which you may send directly to your insurance company, who may reimburse you in accordance with your insurance plan.  Please contact your insurance company for details about payments or any questions about your plan.  

Discounts:

Active and selected reserve military personnel, retirees, and veterans qualify for a 10% discount on all services with full payment at the time of service.  Please bring military ID or DD214 for verification. Thank you for your service!

Cancellation Policy:

Each of our patient’s time is valuable to us and we prepare for this time with us in advance.  Each time a patient misses an appointment without providing proper 24-hour notice, another patient is prevented from receiving care. Therefore, a standard fee of $150.00 for missed return visits and the full fee for a missed new patient visit may be assessed.  This refers to missed appointments (“no shows”) and appointments cancelled less than 24 hours in advance.  Please note that to avoid the cancellation fee, you must speak with our office on the phone; cancellations via email or via voicemail after hours may be subject to the fee.  Thank you for your understanding and cooperation as we strive to best serve the needs of all of our patients.

Late Fee:

A 2% late charge will be added to an account balance 60 days after the date of service.
